About the Phase 1

During this phase, the awarded R&D providers are asked to develop an overall conceptual architecture and technical specifications for each of the system components and their interfaces based on the requirements, use cases and service process models. A series of deliverables will be submitted in order to evaluate the progress based on a predefined set of criteria.

The bids which have been awarded to the Phase 1 Solution Design are asked to develop an overall conceptual architecture and technical specifications for each of the system components, as well as for their interfaces. The Solution Design is based on the requirements, use cases and service process models provided in the Call for Tenders. A series of deliverables will be submitted in order to evaluate the progress, based on a predefined set of criteria.

The Phase 1 “Solution Design” lasts 4 months (October 2022 to January 2023), and it will be followed by a second evaluation of the results, allowing only 4 bids to continue the R&D.

The design of the solutions is based on the previous research conducted within the project, which outlined the importance of:

Stronger coordination and engagement of local key actors is at the core of an improved approach,

Mass Casualty Incidents (MCIs) often cross the national borders and require an efficient and effective cooperation among countries and institutions. Thus, transnational cooperation has been identified as an essential component of properly managing MCIs,

Lastly, innovative and disruptive technologies must be developed and customised to the specific needs of the actors involved, facilitating the cooperation of the field, the communication among the wide range of stakeholders involved in the MCI and supporting the decision-making process of the EMS, in order to react quickly and efficiently during the disaster.

Preliminary Results & Abstracts

In the Phase 1 of the project, the iProcureSecurity PCP team has made major progress towards the implementation of the action foreseen by the Description of Action, evaluating 15 bids, awarding seven contractors, and following up with their work and implementation. The abstract of the solutions prepared as publicly available part of the work implemented by the suppliers, are below:

iTRIAXEMS: The iTRIAXEMS solution aims to address the challenges faced by European Emergency Medical Services in mass casualty events when it comes to improving triage practices, resource allocation, decision making, availability, security, and data protection in a crisis management context. In response to this challenge, we propose the development of a cloud platform which combines software and hardware components that will allow to go a step beyond the current state of the art, due to the application of innovative technologies and functionalities, such as Triage Tag Devices, Drones, Artificial Vision, or AI technologies. The execution of the project will be based on agile methodologies, co-design strategies, and the active involvement of qualified experts. We expect the development of iTRIAXEMS to have a major impact on victims and procurers, and to reduce the number of fatalities and serious sequelae when a mass-casualty incident is declared.

i-TRIER: I-TRIER brings as a legacy one of the very first fully digitized Triage Solutions, widely adopted Crisis Management System and Computer Aided Dispatch systems and a unique toolkit of Hospital Management Systems providing interoperability solutions for unifying Emergency and Victim Tracking data domains with the health-data related ones. i-TRIER brings also to the forefront the citizens through the management of their PHR and by securely providing access to it by EMS. A modular, open, interoperable yet strongly integrated solution will be provided, through advancements in existing functionalities both in hardware and software components. i-TRIER will establish a flexible interoperability framework through an efficient streaming mechanism and local exchange of information. Towards the design and testing of the i-TRIER solution, the technology providers will be supported by experts throughout its development stages, ensuring that KPIs related to the MCI response will be improved.

ENSURE: ENSURE system aims at strengthening the response of Emergency Medical Services (EMS) and Responders (EMR) to Mass Casualty Incidents (MCIs). The latter are called to operate under stressful and uncertain environments, given that MCIs are usually evolved in complex and confusing situations. In the first phase of the project, the consortium is designing a novel digital triage system by utilizing the unique expertise of Satways, Traqbeat and Incisio. By leveraging hands on experts’ experience in emergency medicine (Incisio), the project will combine medical wearables (Traqbeat) with emergency management system (Satways) into a novel digital triage system for MCIs. In a disaster event, the system will enable the quick prioritization of those who need high medical attention by automatically recognizing and continuously monitoring their vital signs via an attachable patch from the disaster location to the hospital.

Reply: Reply for Innovative SCenario in Emergency Development – Design and build an MCI management solution, integrated with external EHR systems to speed up and outperform the continuous handover process of data collection and sharing required from the beginning of the triage process until the hospitalization of the casualties. In order to support the different actors in this process the platform will include the digitalization of procedures and algorithms already in use in MCI management enhanced by a decision support system and by the exploitation of AI/ML modules. The solution will include a “Digital Triage Tag” composed of a smartphone and wearable devices to enable the collection of vital parameters in real-time to build the profile of the casualties and inform their state of severity along the process. The solution is built to support different types of roles and stakeholders of the system and it’s designed to ensure a high level of configurability for each profile. The architecture proposed aims to build a resilient, performing, safe and interoperable platform and will be deployable in a hybrid, cloud-based or on-premises environments, so that it can be easily replicable across different geographic regions.

INSTANT: INSTANT aims to rectify the fragmentation between civilian and military triage systems by i) analysing the strengths and weaknesses of each system currently in use throughout Europe and ii) proposing and designing a unified and modular Triage system, able to adapt to local needs and regulations in place. Identifying and analysing each system will be performed via a systematic search of the international literature using defined keywords and outcome variables, and locally by in-person interviews of medical professionals working at various hospital settings in European countries. Outcome variables such as survivability, need for hospitalization, cost per patient, etc., will be also recorded, analysed and used to define strengths and weaknesses of each Triage system. In addition to classic statistical tools for reviews (forest plots, ANOVA, and so on), data obtained will be modelled using graph database technologies in order to help recording and analysing the complex relationships of the data sets in a more flexible manner.

OMNIATRIAGE: OMNIATRIAGE will deliver a comprehensive solution for mass casualty incident management, based on the OMNIACARE system created by eResult, to support coordinate and efficient rescue operations. Easy tracking and geolocation of each casualty, continuous flow of data in real time about their conditions and advanced data visualization system will constitute some of the main features of the solution, to sustain the management of the medical site on the field. OMNIATRIAGE will further provide support to operations center work, helping to manage human and vehicle resources, thanks to a continuous exchange of data between MCI sites, hospitals, rescue services and control rooms, to optimize the response of the rescuers in critical situations. The solution is designed to be easily implemented to every kind of system in use, thanks to its inherent interoperability and the modular structure that allows high performance and adaptability in different situations and environments.

REACT2-CAS: REACT2-CAS is an innovative, modular and flexible triage management system aimed at increasing the effectiveness and efficiency of EMS services. The system is designed to facilitate the coordination of EMS Services and other partner emergency services, at local, regional, national and transnational levels. REACT2-CAS provides the full functions of a single integrated command-and-control centre in a distributed way: it provides all the key services with the required IT system to support and facilitate effective coordination, communication, decision making and action management, at each level of management or command personnel. REACT2-CAS is deployed as a series of distributed cloud-based services that are accessible via the standard communication infrastructures, i.e., Internet and mobile networks, as well as future systems such as 5G and SatComms. The solution presented comprises of the following eight main components:1. Central information system (CIS)2. electronic Triage Tag3. Data visualisation system (DVS)4. Advanced Medical Site Management5. Clinical Parameters Collection System 6. Operation Site Management7. Vehicle and Resource Management 8. Evaluation and Training.

Next Steps

The Phase 1, which ends in January 2023, will be followed by the Phase 2 – Prototype Development.

Qualified contractors will develop and test prototypes in two iterations. The first iteration (v1) aims at developing non- or partly functional prototypes of key systems components. Test outcomes will be collected and analysed for design, to serve as input for the suppliers’ development of the second iteration (v2). Outcomes of v2 prototype testing will also be used for the evaluation of the phase outcomes. The image below shows the overall structure of the different project phases, as well as the competitive process for the contractors.

The iProcureSecurity PCP project focuses on the development of Triage Management Systems solutions for Emergence Medical Services and has now launched its 6,7 mio. EUR Call for Tenders, following the Pre-Commercial Procurement initiative, which fosters Research and Development of innovative technologies. It allows the Research and Development of innovative disruptive technologies to support the management of Mass Casualty Incidents.

Mass Casualty Incidents (MCI) have been known for a long time and they challenge societies, organisations and institutions. Besides pandemics, societies face natural and human-caused disasters, such as floods, earthquakes, armed conflicts, terrorist attacks or global-scale disasters. In these kinds of events, not only prevention, but also the proper management of the incident on the field can be very challenging, especially for those actors involved in the rescue, triage, treatment and decision-making processes (Emergency Medical Services). But also, a wide range of stakeholders, agencies and institutions are necessarily involved in the management of the event in order to facilitate the rescue operations and the support the casualties involved (e.g., local and national authorities, hospitals, dispatch centers, etc.).

It should be considered, that Emergency Medical Services (EMS) in Europe are characterised by a pluralistic landscape with diverse organisational setups, professional standards, coordination mechanisms and actors which result from different historical and institutional contexts in EU member states. However, diversity is united by the common aim of providing timely care to casualties of sudden and life-threatening injuries, emergencies or disasters in cross-border settings and international humanitarian missions. Fostering the response capacities and increasing the cooperation of the European EMS is of decisive importance for strengthening the resilience of European societies in the light of multiple hazards thus, calling for close cooperation of public safety and health authorities on an international level.

The vision of iProcureSecurity PCP builds the foundation for the development of novel triage management systems, that are able to overcome fundamental shortcomings of currently used systems and which will allow to digitalise key processes and thereby strongly contribute to an improved quality of the service for all involved stakeholders.

Core Research

The research developed as part of the iProcureSecurity PCP project is based on the findings collected and analysed during the iProcureSecurity CSA project and enlarged by in-depth assessments during the first months of the project.

The implemented activities aimed to first understand the gaps and shortcomings related to triage management systems as well as to pave the way for the development of innovative systems able to support the EMS in the operational management of the MCIs. The envisaged solution will enable planning and decision-making processes, taking into account all the existing variables faced by the EMS practitioners in the MCI area. Likewise, the allocation of resources will be more efficient and the cost of each intervention will be reduced, while always ensuring casualty safety. In general, all emergency professionals the project consortium engaged with, claimed that the current practices in the area of triage management need to be improved and the development that is carried out by the industry has to go beyond the current state of the art.

A system that truly has an impact on the work of the emergency teams should allow a quick, easy and uninterrupted communication and information flow among the EMS practitioners on the field, allowing them to exchange information in real time with the other stakeholders in the EMS ecosystem (e.g., information sharing with the Dispatch Centre and the hospitals). Furthermore, part of the project vision is to connect the iProcureSecurity PCP solution with and to existing and upcoming third-party solutions and databases (e.g., casualty’s medical history). The aforementioned necessity implies that the iProcureSecurity PCP Solution is able to exchange data directly with other information systems of the EMS organisations involved. The interoperability concept has to ensure that existing and upcoming third-party solutions can connect to the solution according to clearly defined standards and thereby ensure sustainability of the solution.

A system for triage management that meets the challenges faced by the EMS practitioners across Europe should be digital and able to provide data that facilitates the evaluation of interventions between different teams on national or European levels. However, to achieve this goal, the iProcureSecurity PCP Solution needs to demonstrate also the capability to be used during MCI training.

Finally, as the health data of casualties that is transferred and updated between the different actors is concerned, data protection must be guaranteed at all times and supported by putting in place all the necessary cybersecurity measures.

Methodology and Preliminary Results

In its first eight months, the iProcureSecurity PCP project has made major progress towards its goal of developing the first phase of the PCP procedure.

Requirements, use cases and process models: In order to reach the results, in-depth research and analytic activities have been carried out, based on desk-based research, literature reviews, focus groups, workshops and data collection and analysis. These activities enable the identification of more than 200 requirements; the creation of nine use cases, and ten service process models.

Call for Tenders: All the previous results paved the way for the creation of a comprehensive Call for Tenders, which has been officially launched on the 15th of June 2022, through the official channel eTED Portal and on the dedicated Innovation Procurement Platform, which allows the monitoring and submission of Tenders, as well as the showcase of products and solutions and the matchmaking with other suppliers. (More information below).

Open Market Consultation: The process has been facilitated and supported by the organisation and delivery of seven Open Market Consultation events, which enabled the first communication channels with potential suppliers and a first insights on the technologies available on the market and their level of readiness.

The research conducted within the project outlined the importance of:

Stronger coordination and engagement of local key actors is at the core of an improved approach,

Enhancing cooperation in Mass Casualty Incidents (MCIs), which often cross the national borders and requires an efficient and effective cooperation among countries and institutions. Thus, transnational cooperation has been identified as an essential component of properly manage MCIs,

Lastly, innovative and disruptive technologies, which must be developed and customised to the specific needs of the actors involved, facilitating the cooperation of the field, the communication among the wide range of stakeholders involved in the MCI and supporting the decision-making process of the EMS, in order to react quickly and efficiently during the disaster.

Call for Tenders is launched

The project iProcureSecurity PCP launched the Call for Tenders, opening new opportunities for suppliers working on technological innovation applicable in emergency medical fields. To reach the scope, the quality and the efficiency of the project objectives, suppliers will have to take into account (a) the combination of innovative technologies; (b) organisation and structural component in which the solution will be embedded; (c) the integration of existing national-based systems and procedures; and (d) technical requirements and use cases which have been created within the project implementation activities.

From a technology perspective the focus area includes means to continuously capture and update triage information, the sharing of information in real time among several stakeholders and the gathering and updating of casualties’ vital parameters.

The iProcureSecurity PCP project has identified four main challenges in the triage management system:

Triage tracking: The tracking of triage situations is currently done manually by the actors on the field. This makes it quite difficult for the stakeholders to collect the information for an initial overview, to maintain an updated vision of the situation while the latter evolves, and to connect with and update in real time the other stakeholders involved in the rescue activities.

The iProcureSecurity PCP solution should take into consideration digitalising the existing manual system, and thus, allow to maintain a record of the triaged casualties; updating in real time the data baseline; and to share it with the relevant stakeholder.

Data interoperability: The data interoperability between different organisations on-site is currently a challenging task, although extremely pivotal for the successful outcomes of rescues in major incidents.

The digitalized solution provided by the project wants to provide a fully interoperable system that will be able to react and adapt to unexpected changes in the situation. This will also simplify and smooth the decision-making processes for casualty’s prioritisation and resource planning.

Information handover: Casualties are often handed over to several stations, when major incidents occur. This also means that the casualties are monitored, checked and treated by different actors. The objective for the information handover is to be as accurate as possible, while also consuming as little time as possible for the personnel involved, which can be a challenging task if factors such as a manually written or transcribed documentation, proprietary systems and potentially semantic or taxonomic difficulties are involved.

A distribution of digitalised triage information to any authorised data consumer is efficient, consistent and reliable and does not bind human resources of the involved organisations. It also has the inherent advantage of providing a larger amount of information than what is strictly necessary for the supported process step, which would be well beyond the scope of an efficient manual handover.

Mutable circumstances: In a large-scale incident, the situation can evolve rapidly, involving multiple organisations, carrying out a large subset of routines involving multiple decision points. A manual documentation of these activities is challenging, as it binds valuable resources and is often carried out under stress, impacting the accuracy, thoroughness and correctness of captured information.

The solution will provide a consistent, chronological, documentation on the triage classification, the treatment received on site, and the handover for transportation. By using consistent reference objects and adhering to standardised data formats, a comprehensive data basis is created throughout an incident, which supports the analysis of how the incident situation evolved on site, and derives insights on how to continuously improve the triage procedure from a long-term perspective

Innovation Procurement Platform

The iProcureSecurity PCP project developed the Innovation Procurement Platform.

The platform has been designed to provide suppliers with the option to register their organisations, showcase products and solutions – as well as additional relevant documents – and to match interests, and services with other suppliers and relevant partners.

In addition to that, and following the specifications of the iProcureSecurity PCP project, the online Marketplace for Innovation Procurement included among its features the tender submission and evaluation. The embedded system serves as a transparent, intuitive and easy-to-use evaluation process for tenders and, as such, it promotes also an unbiased selection of offers.

In its initial phase, the Marketplace has launched the Innovation Procurement Platform, which focuses – due to the project scope – mostly on the EMS (Emergency Medical Service) sector.

Following the needs and specifications of the iProcureSecurity PCP project, the Innovation Procurement Platform will serve the different organisations in:

Matching and connecting with other suppliers

Monitoring and checking several tenders

Getting informed, updated and guided throughout the creation of the proposals

Submitting Open Tender Proposals

To address some of these gaps, the iProcureSecurity pre-commercial procurement (PCP) project has been launched, focusing on Innovative Triage Management Systems

A new consortium of 12 leading research organizations, government bodies and emergency medical service practitioners, has been established to procure solutions that improve triage scenarios through a flexible triage management system. Furthermore, the consortium will be supported by an advisory and experts board that will provide additional expertise. Overall, the procurers are responsible for the pre-hospital care of over 40 million people.

The three-year European Union funded project will see potential suppliers (well established industry players as well as start-ups are welcome to participate) going through a three-step process outlined in the PCP communication and associated staff working document to determine the best possible solution for emergency medical services practitioners. The processes are outlined below and further information can be found in the attached links as well as the project’s website.

To achieve a solid basis for the large-scale procurement of iProcureSecurity’s triage solutions, impacts must be analysed, quantified and modelled to understand how and under which conditions the new systems and the services in which they are embedded can deliver what they promise. Throughout the project and particularly during the pilots, the consortium will collect empirical evidence by different means and from different sources to achieve these goals. The purpose of the evaluation in this context is twofold:

To support evidence-based decision-making for first responders as well as policy makers in the iProcureSecurity PCP countries and regions when it comes to large-scale procurement after the immediate project duration,

To contribute to a European and global evidence-based decision-making for procurement of iProcureSecurity PCP systems in further interested regions.

Technology Providers & Suppliers

For technology and solution providers, especially SMEs, a PCP creates the unique opportunity to partner with a launching customer which expands their market competitiveness and brand visibility, but also supports their long-term business planning. In addition, the ongoing collaboration within the project provides an ideal opportunity for liaison activities between industry partners and buyer organisations, which further strengthens and homogenises the market for crisis management-related technologies. We encourage technology providers to visit the project’s website for further information.

Impact

The technical platforms established during the PCP will impact the efficiency and reliability of the triage process by providing the technical means to document the outcome of the initial primary triage and additional triage steps, which the treatment victims receive on site, up to the transportation to the hospital, to create a detailed, continuous digital triage documentation for every victim on site, without disruptions. This diminishes the administrative effort for emergency medical services (EMS) personnel and allows them to focus on their prime responsibilities. In addition, it reduces the potential error rate of manually transcribing information to a digital system, and by continuously maintaining a digital record which documents any procedures or treatments on site. As well, a consistent documentation basis is available to optimise the handover for transportation and hospital care.

The solution shall contribute to:

Digitalization: Boosting the degree of digitalization in EMS

Interoperability: Dramatically improving interoperability between emergency services nationally but also across the border

Consolidating management overview: A drastically new experience for decision makers in and out of the field

Reducing re-triage: Continuously monitoring a victim’s condition and raising an alert in case human intervention is necessary

Partners

Overall, the iProcureSecurity PCP project brings together 12 partners from 6 countries who will integrate their expertise in a variety of disciplines as part of the project’s multi-disciplinary approach. The consortium consists of: SYNYO (Austria), ANDALUSIAN PUBLIC COMPANY FOR HEALTH EMERGENCIES (Spain), SERVICIO MADRILEÑO DE SALUD (Spain), AUSTRIAN RED CROSS (Austria), AREU (Italy), ASL BN (Italy), HRC (Greece), EKAB (Greece), IZMIR MUNICIPALITY (Turkey), AMBULANCE AND EMERGENCY PHYSICIANS ASSOCIATION (Turkey), KEMEA (Greece) and EMPIRICA (Germany).

“The preceding iProcure Security Coordination and Support Action has uncovered the need for better Triage Management Systems to strengthen the resilience and interoperability of Emergency Medical Services in the European Union and Associated Countries. The consortium combines a wealth of expertise and is excited to partner with technology providers & suppliers to deliver a tailor-made prototype along the guidelines of the European Commission for the Pre-commercial Procurement. The overall goal is to drive innovation and ensure sustainable high quality public services.” – Project Coordinator
